Are you eligible for the Disability Tax Credit?
This DTC form is separated into two sections; the introduction and the form itself. The introduction includes the following:
- general information about the disability amount
- a questionnaire to help you determine if you may be eligible for the disability amount
- definitions
- tax office addresses
- how to make requests for previous years
- what to do if you disagree with Canada Revenue's decision about your eligibility
The form itself includes an application section (Part A) and a certification section (Part B). Both sections of the form must be completed in order to be assessed.
