City Teams

CITY TEAMS are structured based on age groupings. Children will transition to new therapists as they move through the teams, and the service delivery model varies based on age; moving from more 1:1 therapy at earlier ages, towards more consultation, planning and group work with older children.

·      Child Assessment Program (CAP); to all newly referred clients, regardless of age and/or location Intervention for 0 to 2½ years

·         Continue to engage parents actively in intervention process

·         Facilitate transitions into elementary school

SERVICES PROVIDED

·      Focus on providing support for establishment of functional skill sets

·      Intervention for child in grade 1 to end of grade 5 (gr. 5 may be consult year) excluding speech & language services for children in self contained classrooms as these services are provided by the school board.

·      Various groups offered on a needed basis

·      Work collaboratively with co-lateral service providers, ex. School Personnel

·      As appropriate, an inter-disciplinary summary assessment at end of grade 5 to lead into applied skill set phase.

·      Occupational Therapy may continue to be active

·      Various groups focusing on community integration and increasing independence with life skills, pre-vocational skills, social skills, self-esteem, leisure

·      Important focus on transition support to high school

·      Inter-disciplinary pre-vocational assessment and support

·      Support discharge planning
